Clinic Forms:
When a student does not have a needed or requested medication at school the following information is required for someone other than the parent/guardian overseeing the medication administration. The parent/guardian must send written permission (FAX will be accepted) and a copy of their photo identification to the school. The person giving the medication must provide identification upon arrival at school. The written permission will be filed in the student's records.
